M.M.J. REGULATIONS
*I am in current possession of a valid Colorado certificate as proof of registration with the
Colorado Department of Health's Medical Marijuana Registry, or I am in the process of
getting it. I understand that services will ONLY be provided to those individuals who can
provide proof of current legal status via this method.
*If you are a legal patient currently without a listed primary caregiver, you may allow
gREENfAITHmINISTRY to assign your primary caregiver with one of our professional
legal medical cannabis caregivers. Although you will normally receive your medicine from
gREENfAITHmINISTRY, you have the opportunity to protect our medicine providers by
establishing a confirmation, in written form, that we are providing for the "Significant
Responsibility for the well-being of the patient.", as noted in Colorado Constitutional
Amendment 20.
*While you may decide, for whatever reason, to leave your caregiver status blank (open),
as registered through the state, you may still utilize the services of
gREENfAITHmINISTRY.
**These services are being provided with the understanding that owners of this entity are
temporarily being asked to act in the capacity of your legal primary caregiver,as defined by
the State of Colorado, during the time frame for which services are being rendered.
*Patient/client agrees, in writing, that, should the need arise, they will make themselves
personally available to testify in court as to the fact that members of
gREENfAITHmINISTRY have provided for the "Significant Responsibility for the
well-being of the patient." (I.E.-you), as outlined in Colorado Constitutional Amendment 20.
*Before beginning your cannabis therapy treatment, please consult your physician for
advice regarding usage and dosage. We are not responsible for any ill or adverse effects
possibly experienced during your cannabis therapy.
*All dispensed medication is 100% organically grown whereas gREENfAITHmINISTRY'S
caregivers agree to not use any chemicals in our raising, and processing of medicines, and
ALL plants must be flushed a minimum of 1 week.
*"Colorado Constitutional Amendment 20" allows the medical use of marijuana with a
physician's recommendation and state registration. Marijuana is still considered illegal
federally.
***I understand that the services provided to me by gREENfAITHmINISTRY have a
solely MEDICINAL/SPIRITUAL focus, NOT being made available for "recreational" use
or abuse, and will NOT be utilized for illegal
purposes of any type.
